You have two different commanders available: Maralen of the Mornsong or Sidisi, Undead Vizier. By keeping the total converted mana cost of all cards in the deck under 40, Ad Nauseam now reads: Pick up your deck and go down to 1 life.
Step 1: Grab Ad Nauseam out of your deck with your commander or a tutor.
Step 2: Cast it, and pick up your deck.
Step 3: Cast Aetherflux Reservoir and a bunch of zero-cost spells to ping all enemies to death.
If you need to go infinite you can use elixir of immmortality with Vampiric Tutor targeting Ad Nauseam again. Use Barren Moor to cycle and draw it off the top.
Unfortunately, the only protection that I could find for the combo is Imp's Mischief. The only real backup plan I could find is Opposition Agent (with Maralen of the Mornsong). At least Boseiju, Who Shelters All can prevent Ad Nauseam from being countered, but the whole deck is still just a meme.
