You could run Mindslaver instead of worst fears. Also, since Mindslaver is an artifact, there are more things you can do to recur it.

Geth, Lord of the Vault might be a fun idea for you. It would allow you to steal creatures and artifacts from your opponents' graveyards.

Lich + Repay in Kind isn't an infinite combo, and will also confuse the crap out of your play group. It will win you games though if you want that.

After doing something like Gary or Exsanguinate, you could drop Wound Reflection to finish off weaker opponents. You could also run it with Temporal Extortion.

You could make yourself lose life (hooray for suicide black!) and then use Magus of the Mirror then go right into Repay in Kind.

Since my favorite card in all of MTG is Sen Triplets, obviously I know a thing or two about Esper and I bet Oloro is running a bunch of counter spells. I bet he also has those two cards that make him win if his life is over certain values, right? Well, tell him to go suck it with Tainted Remedy

Feldon using Academy Ruins? Well you could always Strip Mine that. But why would you want to hurt him? Be his friend! Use Mycosynth Lattice to turn everything into an artifact. I'm sure he'd love that. Then you just drop Null Rod and make it so that no one can tap lands or any kind of activated abilities. Then you just swing with your creatures and win! (zombies from Grave Titan can still enter if he attacks)

Really want to have some fun with commander? Want to be able to tell everyone to sac their permanents without using Eldrazi or Smokestack? Use Phyrexian Obliterator! Probably one of the best 4 drops out there! Just use some kind of Stuffy Doll effect and watch everyone cringe!

Grave Pact or Dictate of Erebos for a not so evil version of the above.

In EDH life typically isn't something you need to worry about unless you are losing it in large quantities and even then, there are funny ways to make that work (Lich + Repay in Kind ). So I would say that it is absolutely worth it. Not to mention, the flavor text on Anguished Unmaking is pure gold.
