Over run your opponent with a gargantuan horde of massive zombies. I actually run a version of this deck. I'm missing my Urborg, but all in all, I have everything in this deck with the exception of one Temporal Extortion.

So, combo and win condition time:

Endless Whispers in play when Phage the Untouchable goes to the graveyard is an automatic loss for one opponent, and good in one on one games. (You can use Miren, the Moaning Well for this.)

Mask of Avacyn + Phage the Untouchable

Cabal Coffers + Urborg, Tomb of Yawgmoth should go without saying.

The power buff guys (Undead Lords) + Phyrexian Crusader to net you at least ten poison counters on the attack.

Grave Pact + Bone Splinters to force a sacrifice and net an extra creature removal. (Good for those pesky indestructables, and protections.)

Nim Deathmantle + Grave Titan to make him a zombie, and give him the benefit of buffs.
