I wouldn't play Creeping Dread. Too slow, too costly and too situational. Castigate is also just a more mana-intensive version of discard like Thoughtseize, but I also like it as it is that much more budget-friendly. Thief of Blood doesn't seem to be Modern legal.I also think that Markov Dreadknight is not very strong here, you only have 7 cards with Madness in total. Maybe just stick to Extort without putting Madness into the deck... if you really want to play Madness, you really need 4 Falkenrath Gorger. Blood Artist, Cliffhaven Vampire and Bloodghast could also be useful for you. Exquisite Blood and Cliffhaven Vampire can create an endless spiral if you like that sort of thing
For Extort, I like Blind Obedience. Slows your opponent down, can net you a free swing and has Extort on it.

Creeping Dread and Markov Dreadknight are really only in there be cause I had nothing better to get the madness effect. I was thinking I might replace them with Falkenrath Gorger and Call the Bloodline.
I'm not the biggest fan of any of the combos with.Exquisite Blood, I feel it's too cheap, so I try to stay away from it. lol
I definitely like the idea of putting a couple Blind Obedience in this deck.

This was previously a deck that focused on pretty much nothing but extort, but when Shadows came out I was really excited to use some of the new Vamps. Also focusing on extort wasn't getting me as many victories as I was hoping for.
I was just testing the madness in there to see if I liked it at all, and I found I did so I'm really wanting to use more of it. I had no idea about Falkenrath Gorger, so I'm glad you suggested it.
I was also thinking about switching out Fiery Temper with Lightning Axe maybe...

Most B/R Madness Vampires run 4 Fiery Temper and 3-4 Lightning Axes. Fiery Temper is basically a Lightning Bolt in dedicated Madness decks. Call the Bloodline is decent discard, Heir of Falkenrath Flip is also really strong, Olivia, Mobilized for War is pretty good as well.
The problem with the new Vampires and the old ones is, that their strategies are completly different. New Vamps are extremely aggressive and fast-paced, the old ones and Extort make for drawn-out games. Most of them don't really mix all to well. Except for stuff like Stromkirk Captain, Kalitas, Traitor of Ghet, Vampire Nighthawk or Gatekeeper of Malakir. Those are just all around great cards in my opinion. There are also Urge to Feed and Feast of Blood if you want to go deeper into a Tribal direction with this.

Okay, so I'm thinking about taking out:
x2Bloodmad Vampire(maybe sideboard)
x2Thief of Blood(btw I knows it's not legal, I just have it cause my friends like to run +1/+1 counters)
Pontiff of Blight(move to sideboard)
And I wanna replace them with:
x2Lightning Axe(I only have 2 atm)
I would really like to use Kalitas, Traitor of Ghet and Olivia, Mobilized for War but their not exactly budget-friendly unfortunately.
I might try to use Heir of Falkenrath Flip someday, but probably not right now.

I think that's a good start, if you play a couple of games with that line-up you'll probably notice if something is missing yourself, or if the deck feels clunky. Luckily, B/W/R has solutions for all sorts of problems. There are lot of upgrades you could go for, but that's a decent start if you want to keep it budget. My favourite thing about Heir of Falkenrath Flip is probably that I can play it on turn 2 and discard Incorrigible Youths on turn 3 to swing for 7. In general, with sufficient Madness cards, Heir to the Night Flip is a 3/2 Flyer for 2 Mana, that's pretty strong overall.
