Surgical Extraction tends to be a popular option for modern sideboard decks. Another option I really like is Spell Snare. It's good vs decks that uses a lot of counter spells like Mana Leak or Remand.

Shadow of Doubt is a really good sideboard way to hose fetches and put someone a turn behind. Especially effective against Abzan.
Dispel is another common SB card to protect your combo. Forces you to run a turn late but it's a safety net.
I'm almost tempted to suggest that the mainboard splash some and go Grixis for Purphoros, God of the Forge, this would reduce your dependence on Altar of the Brood. As a creature spell he could be cast for with Heartless Summoning in play.
All in all a really cool deck, I love it.

@Nefferean Surgical Extraction is pretty good, but it isn't particularly strong against any of my bad matchups (basically control). Spell Snare would be a good option though, might have to add it in.
@Ohthenoises Shadow of Doubt would be really good actually, slowing people down a land-drop is pretty relevant and it can also help my Scapeshift matchup.
Dispel might actually be better than Spell Snare as a sideboard card, it does slow me down but having guaranteed protection is very nice.
Also I'd rather not move this into Grixis. The original Heartless Summoning decks did that and it had a very punishing manabase, plus it could run Purphoros or Grapeshot. However, Altar of the Brood is cheap enough that we can play it for free by using Myr Moonvessels mana, so we can combo with a Grim Haruspex out and win on the same turn. It is also tutorable with Trinket Mage, which is another benefit. Purphoros, God of the Forge might be harder to deal with, but he is too slow for a deck that has to win by turn 5 or else it loses. Altar of the Brood is also kinda nice since it messes with other people's decks, I can see their cards as they mill so I know what to expect for games 2 and 3, and I can also potentially mill them out given enough time and a couple altars.

@Nefferean This is true, it hits a lot of relevant creatures like Goyf and Bob. However, I can easily outrace most decks that play Tarmogoyf, since they are typically focused on removing creatures and this deck doesn't require any creatures to be in play to win.

Yah I was thinking about that after I posted it lol. I agree with Shadow of Doubt because it can be pretty devastating in the early game against a fetch.

@ChiefBell That is actually a pretty good idea, the scry makes it worthwhile since it also digs for my combo. I'll probably throw two of these in.
Ok, I think overall that my sideboard will have -1 Nihil Spellbomb, 2 Drown in Sorrow, 2 Shadow of Doubt, and 1 Venser, Shaper Savant. I'll need to go and playtest this to make sure everything works out, but it looks good on paper.

I've seen some builds of this deck use Court Hussar over Mulldrifter to increase the dig capability. Any thoughts on that? I'm considering building this myself so I wanted some advice from someone who's had experience running it already.

@Rinne I've actually never seen Court Hussar, but it is very interesting. I could definitely see it instead of Mulldrifter, although I like having Mulldrifter in the cases where I have a terrible starting hand and I need both lands and combo pieces. I guess you would need to run playtests to see if digging one card deeper is worth it more often than drawing an extra card.
I might actually look at swapping out Trinket Mage in the main for a Court Hussar, since it's dig seems more useful when we don't already have our combo. If it performs well, then I'll probably do a 3/2 split of Mulldrifter and Court Hussar

@marioman77 That is why I have Laboratory Maniac in the side, this way we can kill them by drawing ourselves to death. Also we can use Havengul Lich and Perilous Myr to shock them to death.

@Nefferean Telling Time is good, but not in this shell. We need to win as fast as possible, and expending resources on turn 2 to dig is too slow. We ideally want to be digging turn 1, then playing Heartless Summoning on turn 2.

@CastleSiege I haven't, mainly because it costs too much for the value that it would be giving us. I couldn't possibly get any value out of it until turn 4, and it is only good if Mulldrifter, Court Hussar, or Trinket Mage are under it. The tokens are exiled, which stops it from being good with my Myrs. I'd rather just run more copies of those cards instead of running this.
