You have a point on Windfall . I removed Rhystic Study to make room because it seems underwhelming with all the wheel effects.
I'll also remove Pyroblast for Library of Leng .

Nekusar, the Mindrazer only triggers when an opponent draws a card, not when its controller draws, so I fail to see the interaction between him and Consecrated Sphinx .
However, Sphinx does have synergy with Psychosis Crawler .

I get to draw, roughly, 8-12 cards between my turns, then I ditch them all either when I play a wheel effect, reach my discard step without having dug to a Reliquary Tower or Library of Leng , or to the guy in my playgroup that likes to recur discard effects.
I'm aware of the power of Consecrated Sphinx
, but it's not an auto-include in every deck running blue. It has synergy with parts of my deck, but in my meta it's likely to either get countered, or bounced/stolen/killed before it can do any good.
The theme of Nekusar, the Mindrazer is to force your opponents to kill themselves when they draw cards, and Consecrated Sphinx doesn't follow that theme because its effect is one-sided.

Great build I love Nekusar decks, mine is my favorite EDH deck to play out of my collection. You can take a look for ideas if you want Euthanasia (Creature-less EDH)

A few cards that you can consider are Megrim and Liliana's Caress . With all the discarding you will have your opponents doing it can really add to the damage. With all the cards that your opponent will be drawing even cards like Iron Maiden , Skullcage , and Viseling can add to the fun. Black Vise is also a good choice, but it only targets one opponent. I have a five color deck similar to this that only uses wheels and wrath effects, check it out here everyone draws cards. +1

Viseling and Iron Maiden don't work well with Sire Of Insanity . If they triggered in the Draw or Upkeep phases I would include them, definitely. Skullcage might work, though.
I've considered Liliana's Caress , but I don't know what to remove. I don't know about Megrim because the damage can be prevented or redirected, effects that are somewhat common in my meta.
What would you suggest I remove to make room for those?

If you don't like Viseling and Iron Maiden because of Sire Of Insanity , then how about Wheel of Torture and Rackling . But with all of the every player draws cards spells that you are using, without sire out, the viseling and iron maiden would end up dealing a ton of damage. As for Liliana's Caress , i would remove 1 mountain. With all the wheel spells, you will be resetting your hand enough where you will probably hit every land drop after the first 3 or 4 with no problem.

I don't like how conditional they are, not necessarily their effect. I think they work better with a build that lets everybody have no max hand size (Price of Knowledge and Anvil of Bogardan effects) and probably runs Forced Fruition to keep their hands big.

nekusar + underworld dreams + spiteful visions + Cerebral Vortex + a few copy spells - you draw 3, opponent cries.

I don't like Words of Waste
and Words of War
due to their mana investment.
I've considered Phyrexian Tyranny , but since I have the ability to draw TONS more cards than my opponents I run the risk of killing myself or being forced to tap out.
Cerebral Vortex looks fun, but I don't like that it only affects 1 player.

What about Molten Psyche ? It synergizes well with your deck, you already have a good metalcraft enabler in 20 artifacts.

I had Molten Psyche
in the original build, but I didn't like how conditional it became. To get the maximum effect, I had to wheel into it while also having 3+ artifacts on the field. It needs an almost Storm-like condition of events to get value.
I also didn't like that it shuffles the cards back into the deck, which doesn't really provide a lot of disruption in a format with so many tutor effects.
Winds of Change gets a pass because it can be cast turn 1 or 2 to disrupt opening hands, and it's a cheap way to tuck creatures that have been bounced.

Looks good, but just to clarify, "Commander Damage" is combat based, so damage that would be dealt as a result of damage Nekusar would deal to your opponents life total after/with no blockers assigned.
If a player has been dealt 21 points of combat damage by a particular Commander during the game, that player loses a game. Details
This is an additional state based effect.
Commander Damage is cumulative throughout the game; nothing can reduce the amount of damage a Commander has previously done to a player.
Because it is a property of the card and not a characteristic of the game object, a card is still the same Commander even if it leaves the field and returns.
While effects can raise a player's life total, it doesn't reduce the amount of damage previously taken from a Commander. (eg: Beacon of Immortality)
Conversely, combat damage can be reduced, prevented, or replaced as it is taken, in which case it was never dealt and doesn't count towards the total taken from that Commander. (eg: Fog or Captain's Maneuver)
Commander Damage is specific to each Commander/Player pairing, not combined across all Commander.
A player can lose if he or she is dealt 21 points of combat damage by his or her own Commander (ie: under someone else's control).

Glistening Oil is a quick way to finish out a game if the opportunity presents itself. Death by infect can be done with just two wheel effects.
Helm of the Ghastlord
can provide card advantage, and is especially brutal on Nekusar.
Elder Mastery seems awesome, but I've found that I don't want to cast it because it draws an insane amount of hate and my build doesn't have a lot of protection. Wound Reflection or Exquisite Blood might be better.

Only reason I have Glistening Oil is because it doesn't create a 2-for-1 when the enchanted creature is killed. I don't use Grafted Exoskeleton for the same reason.
I don't like infect, but it helps against life gain decks. That's why I only have 1 source of it, whereas some lists also run Phyresis and Tainted Strike .

Archmage Ascension . I know its a little slow, but your commander satisfies the quest requirement and if it goes off you essentially get to look for two cards of your choice every turn, plus whenever you play a draw spell.

Archmage Ascension is too slow for my liking. I don't typically draw a lot of cards on my opponent's turns without some set up, and I would draw a ton of hate if it gets online.
Tomorrow, Azami's Familiar is something I'm seriously considering, though. It would allow me to sculpt my hand when I wheel instead of drawing 7 random cards.

I want to stay away from designing around infinite combos because my playgroup frowns on them (for now).
I had Windfall included, but I feel there were too many wheel effects and not enough reactive spells, so I cut it. I feel the same way about Wheel of Fate .
Library of Leng is something I'll consider.
