Skullbriar, the Walking Grave swinging out of the gates!

Focusing on evasion, token generator, and sacrifice sub themes to get damage through, avoid an opponent's effects like Grave Pact and Butcher of Malakir, and have the ability to return Skullbriar to the command zone if he will be returned to hand/library where he will lose his +1/+1 counters.

Reliable evasion is provided in Rogue's Passage, Power Matrix, Shizo, Death's Storehouse, Zombie Master (who also protects Skullbriar), Neurok Hoversail and Trailblazer's Boots.

Plenty of pain cards too just to make sure Skullbriar has the damage needed to get through, like Loxodon Warhammer, Rancor, Ring of Kalonia , Fallen Ideal.

Just pump him up with the obvious +1/+1 generators, Increasing Savagery, Strength of the Tajuru . Forgotten Ancient and Corpsejack Menace are some of my favorite cards for this.

Token generators are great for attack or defense, always helps to have chump blockers since Skullbriar will be tapped 99% of the time. Symbiotic Wurm , Necrogenesis, Grave Titan, Avenger of Zendikar, Verdant Force can all start to put hurt on people or help control attacks. Nim Deathmantle can also bring the creature cards back to the battlefield to be sac'd out again or to maintain board presence.

Tokens are great blockers, hang around, take advantage of Fallen Ideal and Skullclamp, Death's Presence and help keep Skullbaby in the game vs Eldrazi annihilators. Also helps give me board presence and control when my own Grave Pact or Dictate of Erebos is on the field.

Momentous Fall, Homeward Path and Grim Backwoods can get Skullbriar out of the way in a hurry or return him back home if he's going to be stolen/tucked/returned to hand, which will probably happen. Disciple of Griselbrand helps here, even gains me a bit of life, something the deck needs. Attrition hangs out on the side, ready to sac Skullbriar (protect vs returning to hand) or tokens for reusable spot removal.

A couple graveyard hate cards thrown in to ruin a reanimator's day, Bojuka Bog and Necrogenesis which also helps make tokens/chump blockers.

Cast the Skullbriar, the Walking Grave , play the ramp cards into higher CMC power-houses and watch the sparks fly!
