Substitute Think Twice for some of your spot removal. It allows you to draw if you're opponent doesn't cast anything and maybe your waiting with a Mana Leak .
It seems to me you should rethink your Doom Blade , card:Geth's Verdict, etc to capitalize on the threat that Phyrexian Obliterator presents. Don't you want creatures to hit him after all?
You could add Bitterheart Witch & card:Curse of Death's Hold with 4x card:Altar's Reap to work with Massacre Wurm . It's kind of slow, but eh. Just an idea.

I would be careful with your creature selection. All of your creatures fall under the wrath of Dismember although a lot of people don't play them anymore because they are useless vs titans some people still side them.
It almost seems like you're floating towards a Solar Flare type of build...not sure really. Here is my deck:solar-flare-help-build-standard-fnm version of Solar Flare to peek at.
If you're not wanting to push that far away from your deck design I would definitely side board the Go for the Throat and run more Think Twice . I've never been a fan of card:Geth's Verdict for removal. With black and blue you have so many "I get to choose what dies" card that it seems pointless to let your opponent choose...once those are out you can bump up your Dissipate and maybe add another Despise in because that is amazing with Surgical Extraction .
You need to get a Grave Titan or two I think for this deck...it gives you at least one creature out of the 5 toughness range and they become a pain for your opponent. Toss in an Unburial Rites (these are awesome if you do move towards a Solar Flare type build) to bring them back if you can't counter your opponents removal.
I personally didn't find any use for the Liliana of the Veil it interferred with cards I wanted to play more than it helped and was always a huge target. Play with her to see what you think but I wouldn't have her in your deck as a one-off card you would need at least 2 or 3 if you can afford them to build that important consistancy.
If you can afford to I would also get your land up to at least 25...I know it seems excessive but for your first 6 turns you need to be dropping a land. Some people might even argue 27 ;)

I have been thinking about upping that land and I agree on the Liliana. I've been thinking of removing some spot removal for a counter spell or land. The Titan was prginally going to be in here but I don't feel like buying any lol probably will trade for 1 or 2 and add. I have a few ideas I'm toying with but will just have to playtest

Alright so my first thought is pitch the Phyrexian Rager , the pay 1 life cantrip really isn't worth it imo, instead run Think Twice as something to cast during their end step if you didn't counter or kill anything. Speaking of counters bring Dissipate up to a 4 of, exiling things is very important these days. Control decks always want more land than less, so I'd say 26 with minimum 2 Ghost Quarter since you're expecting Wolf Run Ramp. Massacre Wurm is cute and all but Wurmcoil Engine is strictly better. If you're planning on seeing a healthy number of Dismember s then I'm afraid that playset of Obliterators is going to being BBBB wasted and Grave Titan is your man. But if you don't expect that then keep em around. I also think you could lose the Ponder s but that's just based on my own preference.

Through research on top tournament decks, I'm not really worried about dismember. I also think the Massacre Wurm works better with the overall idea of the deck as well. The Dissipate s would defeat the purpose of my Surgical Extraction s. Do want the titans though

Surgical Extraction is good against control, it takes out their Dissipate s, so it takes out their only hard counter. So long game=win. Phantasmal Image becomes another one of your beatsticks, or it can let you sink down to your opponents field, so it's a good choice in here(at least 2 sideboard?).
Dissipate is an amazing counterspell in this meta. It Apopa anything dead in it's tracks. Flashback isn't as good anymore, recurring creatures lose there value, and graveyard based decks suffer.

That's the main reason I play Surgical Extraction and no Dissipate s. The majority of deck I plan to see I know when I need to save my leaks to use my extractions. Phantasmal Image would be a good sideboard edition and I'm looking to change it up a little bit to fit it in. I'm thinking of removing one Evil Presence and a Steel Sabotage to place two in the sideboard for cards such as Thrun, the Last Troll or other cards that give trouble removing. The Main addition I want to make to the mainboard is 2 Grave Titan . I think once those make it into the deck it will make it a bit more solid. I playtested against the winner wolf run of worlds and was easily able to hold my own unless I missed one or two key removals.

Wow this is one solid deck build and very lucky I play tested it against my Jaceful Illusions deck, and you were down to 2 cards in library. 9 life Me at 6 life. had a Phyrexian Obliterator with card:Inquisitor's Flail and top decked a Go for the Throat with my lone creature Phantasmal Bear out. The card:Black Sun's Zenith is so great in here as well. I think its pretty damn well built, it can stand against a great many decks I think, I will play test against my other standard deck next

Lost in turn 9 to my G/W Token deck What can Myr do for you!? perhaps a Ratchet Bomb to counter them? Just never drew the Zenith cards. Also for the self mill decks run some Nihil Spellbomb for cards like Kessig Cagebreakers and the like

The 3rd Zenith is in your SB and the Playtest mechanic on TappedOut (that I know of) doesn't take into account it. Also I only played a quick single game against my actual decks not play testing both of them lol
