Do you want to be a control deck or a token deck? Your lands really confuse me for this reason. Colonnade tells me you want to the a control deck, but you're only running 22 lands. You will not get any sort of activation out of Colonnade with that many lands. Unless you can decide what you want to be I can't really help you out much.
If you want to be control with YP/MM as your wincon, which is probably the best way to execute the idea you're going for, add more lands and take out Boros Charm. 4 leaks and 4 paths is a bad idea in general, as it gets much easier for the opponent to pay for the leak if you've given them two extra lands; run 6-7 total. Spell Snare is better than Remand in control, as remanding something basically does nothing for you if you're not going to take a proactive stance. Silence and similar effects are also bad in this shell, as you're, again, not taking a proactive approach, so I'd run something other than those.
My suggestion is: -4 Boros Charm, -4 Silence, -4 Render Silent, -1 Mana Leak; +2-5 Lands, +4 Lightning Helix, +2-3 Spell Snare, +1-2 Cryptic Command.

Thank you for your response Spirib, i'm going to drop the token thing, go full control and add some lands.Which cards would you recommend instead if my creatures?

Daxos of Meletis isn't the best for control. You want a creature with a lot more evasion. For example, in standard, Pearl Lake Ancient is an auto-include because it is a beater, can't be countered, and can be protected from basically everything with its return ability. Another example is Morphling.
However, i believe most control decks in modern use manlands, like Celestial Colonnade, to beat people to death.

Glad to see you're making improvements.
Take out the Cyclonic Rifts, if you want to get rid of creatures en masse, something like Wrath of God or Supreme Verdict is better. Probably want 1 main 1 side depending on your meta, if there's more creature based you might want more.
Snapcaster Mage should be a full playset, it's just too good. Maybe a Keranos/Batterskull in the main deck would help as well, if not in the main then in the side. Also consider Electrolyze for Think Twice. If your meta has a lot of Lingering Souls or BW Tokens, I think it'd be better since it allows you to rival the value from Souls.
Your SB also seems like it could use some work. Deflecting Palm, while nice, isn't going to block more than 4 damage most of the time. Grafdigger's Cage also doesn't hit much now that Pod is gone, so I'd recommend taking that out as well. Timely Reinforcements, Ajani Vengeant, both the good Elspeths, and maybe Combust are nice cards that could be put in there depending on your meta. Also some counterspells should be in there like Negate and Counterflux
Suggestions: -4 Cyclonic Rift, -2 Daxos of Meletis; +4 Snapcaster Mage, +1 Supreme Verdict/Wrath of God, +1 Keranos, God of Storms/Batterskull.

The deck is looking pretty good so far, although the mana could use a little work. something like this might be good (you can also add ZEN fetches if budget isn't an issue) -
- 4x Celestial Colonnade
- 4x Flooded Strand
- 2x Hallowed Fountain
- 2x Steam Vents
- 2x Sacred Foundry
- 4x Island
- 2x Plains
- 2x Sulfur Falls
- 4x Tectonic Edge
I can explain the mana base in more detail if you would like.
Aside from the cards that have already been suggested, some nice SB cards might include - Engineered Explosives, Valorous Stance, Geist of Saint Traft, Vendilion Clique, Stony Silence, Chalice of the Void, Dispel, Leyline of Sanctity, Relic of Progenitus, Spellskite, Anger of the Gods, Aven Mindcensor, and Izzet Staticaster.
If you need help with anything, just let me know :)

You have no mainboard win conditions, save Colonnade. This is a huge problem.
Batterskull is great against aggro. Keranos, God of Storms will win grindy games. These are really the only options in these colours.
Use Shadow of Doubt. It's busted. Snipe a fetchland, search, or just cantrip.

So this is my first modern deck, but there are some points to improve! So help is very appreciated and every comment is welcome!
My thoughts:
1) Control the opponent with a hole punch of instants (specially Silence, Path to Exile and Render Silent
2) Tokenpower with Monastery Mentor and Young Pyromancer
Maybe some suggestions for a final hit like: Token Token Token + Hour of Reckoning
