There are a couple of cards that have similar effects, like Demonic Pact, but of course those have the options on the card itself. Really, it's not too much more difficult to track than a Dungeon, you just need a sheet of paper or something. The shuffle clause is meant to prevent blink shenanigans so that it's a bit easier to track and less easy to abuse.
And technically it does have the chimera subtype, since it's all creature types ;)

I don't think it's a good idea if I absolutely need paper in order to track that information. Remember that both players would need to track that in more official play, and so making it that everyone needs to lug around more out-of-game items and spend all this extra time with notes is annoying to say the least.

You don't necessarily need paper, either. In most competitive play this wouldn't be sticking around more than an absolute maximum of 4 turns, so it seems unlikely that you would begin forgetting what creature types you've already chosen.
Besides, the main place that these custom cards would be played would be in an online format where it's much easier to track these things, and notes can be put on the cards themselves.

This is a neat card, I could see this being good in a tribal-tribal deck or one that relies on Maskwood Nexus. The cmc is a bit high for me personally to actually play but I think at 5/6 cmc you could use it as a Shamanic Revelation or Distant Melody in a normal tribal deck for one time mass card draw. Then with lords in play it would then act as a body towards that tribe which is nice.

I'm not sure if that works in the rules of the game. Plus it can become a Nightmare to track But I like the card. Only thing I might tweak is I'd give it the chimera sub type
