Assistance with Modern Simic Graft/Evolve deck (Or standard Simic deck)

Hey forum. First time actually constructing a deck, and I thought I'd go for simic. Usually I netdeck, but I wanted the satisfaction of winning (hopefully) with a deck I actually made, so I made this one: http://tappedout.net/mtg-decks/simic-modern-graftevolve/
It relies mainly on graft/evolve to keep counters circulating, with a Tumble Magnet and Gilder Bairn combo to double them periodically. Mirrorweave is insane, allowing a clutch victory if you have a single Biovisionary and three other creatures, acting like a semi Wrath of God when you target one of your 0/0 grafters, or making things a copy of Primordial Hydra , allowing them to double their counters like the hydra does. Plaxcaster Frogling adds a cheap 3/3 body with a great hexproof ability. Gyre Sage can pump out insane amounts of mana if needed. Any suggestions for cards/substitutions would be greatly appreciated. Additionally, any suggestions on standard cards would help, although modern is what I'm looking to play, simply because I can use Gilder Bairn . Fear the frog costume!

@jminute14: Biovisionary 's ability is triggered, not static. It would depend on whose turn it is.

For that wincon, you would Mirrorweave at the end of your second main, or during your end step before state-based effects resolved.There is also an option to just put in a Helix Pinnacle for funsies if in need of a backup-backup wincon.

State-based effects don't resolve in the traditional sense, and they can't be responded to. Biovisionary 's ability won't even trigger unless you control four permanents named Biovisionary at the beginning of the end step. You have to cast Mirrorweave during or before the second main phase.
Also of note, if your opponent controls four or more creatures and you Mirrorweave Biovisionary on your turn, you lose. Your triggered abilities will go onto the stack first, followed by your opponent's. His or hers will resolve first. You need to Mirrorweave on your opponent's turn if he or she has four or more creatures.

If you Mirrorweave d Biovisionary and they had 4+ creatures too wouldn't it end in a draw?
