The first installment in the story of Kaldheim has been posted!
I see that the story of Kaldheim has started intensely! The monster in the cave was not clearly identified, but some readers are suspecting that it was Vorinclex or another Phyrexian. I really hope that the plane of Kaldheim can have its own identity, since this is the first visit to that plane. Personally, Kaladesh would have made more sense as a target of a Phyrexian invasion, with its focus on artifacts, but I hope that WotC knows what they are doing with this set.
What is Kaya is even doing on the plane, at all? Why did she go there? I hope that that mystery is solved, soon.

How can the Immersturm be on Kaldheim? Does it exist on two planes simultaneously, or do Kaldheim and Valla coincidentally have similar phenomena?

Kaldheim replaced valla.
This isn't the first time a plane name from planechase got changed.
I belive it was called monseng in planechase, it is now tarkir, and I think the same thing happened with a plane being renamed to theros.
Pretty much what happened is planechase didn't have as many hoops to go through as a set taking place on a plane, so the mini creative team pretty much made up whatever names they wanted for places that didn't exist yet. This can create issues (pretty much translation, language barrier, and legal stuff) so when the plane is fleshed out and designed in full some things about it will change. Such as valla→kaldheim

Monster in the cave is definitely Vorinclex, just re-read the description given while looking at the card. It's described down to the tuft of pelt across the shoulder area behind the head.
As for Kaya, I think the better question is who sent her there as opposed to why is she there. I feel the why is very clear, she has been paid to kill Vorinclex. The important question is who funded the request, especially with the coin she received being from several different planes.

I haven't read the story yet, but perhaps there's a chance Elesh Norn paid her? I wouldn't be shocked if the Phyrexians didn't have their own unique form of currency, but of course they have technology advanced enough to replicate any existing form, as well as knowledge of other planes to boot (at least in the case of Norn). We've known for a while now that Elesh Norn is interested in eliminating her fellow praetors, so I wouldn't be surprised if she sent Vorinclex to Kaldheim on a fabricated 'mission', where she knew that he would be ambushed by Kaya and killed.

I think the two big questions here are:

How did Vorinclex get to Kaldeim?

Who hired Kaya?
A phyrexian travelling planes either means Tezzeret is involved or that there exists some other method of inter-planar travel. Tezzeret does have control of whatever remains of the Infinite Consortium which would give him easy access to the currencies used to pay Kaya. If it is Tezzeret pulling the strings then he most likely used a proxy to deal with Kaya.
DemonDragonJ, I share some of your concern for what a phyrexian's presence can mean for a plane. At least so far I suspect that Vorinclex is alone and we're not looking at an invasion yet. After all, Mirrodin's invasion wasn't depicted until our second visit. I actually hope that Kaya is successful in either killing or neutralizing Vorinclex so that Kaldeim remains safe.

I would argue the "not clearly part". The monster in the cave was described as looking like the new vorinclex card.
