In the original Nicol Bolas card art, it depicts the elder dragon gripping a book with a five fingered hand, but in the art of Chandra's Defeat, it only shows him with three, what happened to him? Is it childhood trauma which made him so self punishment?

Well since we know that Planeswalkers have the ability to change their forms maybe that's what happened. Maybe he chose to have three instead of the full five.

Tetsuo Umezawa defeated Bolas by severing his mind from his body and then killing the body. I don't know the details of how Bolas regained a physical form. The simple answer is that his current body is not his original body.

His design has been reworked since his first artwork. His horns and wings are different, too, and his snout is significantly less pronounced. While planeswalkers did have the ability to shapeshift pre-mending, that isn't what's happened here, as they no longer can. They just hammered down a more consistent and pleasing design for him.

Ok, this is the OFFICIAL lore on the subject:
Between Bolass time as almighty evil dragon and current badass dragon, he started working in a butcher shop to afford the shiny metal coating. One day, while serving up some delicious Tarmogoyf sausages, he was being shouted at by the boss about how he didnt clean the meat grinder properly. He wasnt paying attention and then realized he heard some crunching and felt some sharp pains in both hands. He realized to late that those sausages were now dragon meat and goyf mixed. His boss then fired him, and the butcher shop was soon set on fire.
