when a pemanent leaves play does it lose all its counters?

in another question i read someone answered that they keep the counters even if it goes to the graveyard i thought they didnt so i want to clarify this also is it the same for exile/ remove from game?

If a permanent leaves the battlefield, it loses all counters, unless otherwise specified, as with time counters on suspended cards and the like.

also not with phasing, correct?

Arcbound Worker is the exception. modular shifts counters around when it dies. lol new deck idea

with phasing the permanent technically never leaves the battlefield, so it retains all counters and enchantments upon it.
